What can I expect at my consultation at iSpine Pain Physicians?

At your consultation you will meet with two providers, a mid-level (Physician Assistant or Nurse Practitioner) and a physician. The mid-levels and physicians work as a team to co-manage a patient’s care. You will compete your new patient paperwork ahead of time on our patient portal or by checking in early to your appointment. You will need to bring your photo ID, insurance card(s), and medication list when you check-in. It is important to make sure we have received your medical records by your appointment date.

Once you check-in, you will have some additional paperwork to fill out and then complete the first time check-in process. A clinical assistant will call you back and complete the rooming process, which includes taking your weight, measuring your height, and taking your vitals. They will also have some questions for you about your chronic pain.

The mid-level will come in and begin the consultation. They will be reviewing your medical records, new patient paperwork and ask about your chronic pain symptoms and progression. He or she will also perform a brief physical exam as well. The mid-level and physician will meet to review all of this information and start to formulate a plan of care. Finally, the physician will come into the exam room and speak to you.

At the end of the end appointment you will be given a plan of care, which will outline our providers recommendations based on your medical history and the consultation appointment.

If you are referred out for modalities such as physical therapy or chiropractic care, we will fax out a referral once the note from your consultation has been completed.

You may be asked to come back for additional appointments. For all office visits you will meet with the mid-level and procedures will be completed by the physician.

It is important to make sure that we have your medical records at the time of your consultation. Please have the following records faxed ahead of time or hand carry them to your appointment. Failure to do so may result in a delay in determining your plan of care and/or treatments. Please provide iSpine Pain Physicians with the following records: 3-5 most recent office notes pertaining to your chronic pain, OR notes from injections relating to your chronic pain, imaging reports (most recent MRI/CT/X-ray and EMG), physical therapy notes, and chiropractic notes.

We make every effort to see you at your scheduled appointment time. However, due to the complex nature of diagnosing and treating chronic pain patients, we may be delayed at times. Please be assured that our staff will always take the necessary time to attend to your needs.
