Advanced gum disease, commonly known as periodontitis, requires in-depth treatment to prevent worsening effects. Scaling and root planing are two procedures used by dentists to address problems like tartar buildup and gum recession. They're sometimes performed during the same appointment and are effective at deep-cleaning the teeth and encouraging gum tissue to reattach to tooth roots. Here's a closer look at what to expect and why this treatment is necessary.

What Happens During Scaling & Root Planing?

Your dentist may perform scaling and planing during the same visit or break them into two separate appointments. Scaling involves scraping tartar—which is hardened plaque—from the teeth using special tools. Plaque may also be removed from under the gumline if pockets have formed. These pockets form as gum tissue recedes from the teeth, as often occurs when advanced gum disease is untreated.

Root planing will occur next. Using the same tools, the roots of affected teeth are smoothed to prevent plaque and bacteria from collecting again. A smoother surface also facilitates better healing and reattachment of gum tissue, slowing down or preventing further gum recession in the future. Medication may also be applied directly to the gums to prevent possible infection.

Who Needs This Procedure?

Scaling and root planing are recommended for those in advanced stages of gum disease. While the effects of gingivitis can be reversed early on, by the time gum recession has occurred, the effects are usually irreversible and can only be managed. Symptoms of advanced gum disease include red, swollen gums, blood when brushing, gum recession, and shifting teeth.

What Are the Benefits?

Gum disease can penetrate deep into the jawbone and damage connections between the tooth root and bone tissue. This causes teeth to become loose or even fall out in severe cases. Over time, the jawbone will begin to lose density as well.

With scaling and root planing, the risk of tooth loss can be reduced. The procedure may also relieve the discomfort caused by swollen, irritated gums.
